| Case Management |

| Case Management is the cornerstone of the Community Support program. The Case Manager is responsible for each of their client’s services. Duties of Case Management include; preparing the Personal Centered Plan, assessing needs and coordinating services, linking clients to required services, 1st response to crisis, weekly face-to-face contact, supervision of the Direct Care staff, general advice and counseling, and others. Our case managers are educated, trained, and experienced to handle the needs that our clients have, through our various resources and contacts to the appropriate service or program. |
