Adult: At Advanced Placement, our Adult Community Supports Program offers a comprehensive array of services designed to promote wellness, recovery, achievement, and independence. Our qualified professionals assess the unique needs of an individual, as well as coordinate services based on these needs including long-term goals set to obtain future hopes and dreams. Some of the services that our professionals link individuals to include outpatient psychiatric and therapeutic services to include evaluations with medication and symptom management, individual therapy, substance abuse treatment and recovery programs. We also assist with locating services for medical, dental, employment, transportation, adult education, financial assistance, and housing.
Access to community activities and integrating an individual back into the community is also a must, to further improve a person’s quality of life. Each individual person is unique, so we strive to assist with achieving one’s goals in the most quality person-centered manner obtainable to ensure that long-term success is achieved and triumphs become possible!
Children and adolescents: Community Support Services for children and adolescents sets a precedent for reunification of youth and their families through outstanding one-on-one services in school and in the community. These services are directed by quality, professional case managers highly experienced in working with at-risk and/or troubled youth. We offer a vast array of child and youth services to include, case management, intensive in-home treatment, comprehensive therapy for the individual and family, linkage to psychiatric services and community youth resources, diagnostic assessments, residential treatment (level III), 1-on-1 direct community care, and much more! Each individual receives expert screenings and assessments to determine their unique needs, and then the appropriate services are coordinated and delivered in a person-centered approach with family involvement and overall wellness as a central focus.
The key focus of our services is to transition today’s troubled youth into motivated, self-confident, productive adult men and women who will have been provided these resources in their childhood to make this transition not merely a dream, but a successful reality!
